We are looking to engage highly skilled Complex Case Managers / Clinical Case Managers to deliver trauma-informed case management services. In this role, you will manage a caseload of clients with complex needs, providing high-quality support and facilitating access to health, mental health, and psychosocial services. This is an initial 3-month contract, with potential extensions, based across multiple locations Australia-wide. Flexible working arrangements are also available. The successful Complex Case Managers / Clinical Case Managers will be responsible for, but not limited to: Undertake intake and risk assessments and develop tailored wellbeing plans. Provide specialised case management and clinical advice to enhance service delivery. Build and maintain strong relationships with internal teams and external stakeholders, including community organisations and service providers. Escalate high-risk issues and contribute to high-level client summaries and briefs. Act as the primary point of contact for allocated clients, coordinating access to services and supports. The successful Complex Case Managers / Clinical Case Managers must have: Tertiary qualifications in Nursing, Psychology, Occupational Therapy, or Social Work. Professional experience working with individuals with complex support needs. Strong understanding of disability impacts and capacity-building models. Excellent communication skills and proficiency in IT systems. Experience in mental health services and familiarity with evidence-based therapeutic interventions will be highly regarded.
